It it not necessary to update us every time you attend a medical professional, as long as you send us all the information every three months. To obtain your treatment records we will need the following. Failure to produce any of the following jeopardizes our chances of reaching your best outcome.

• Date(s) of treatment.
• Office name.
• Medical provider’s name (doctor, nurse practitioner, therapist, etc).
• Address of the office where you were seen.
• Phone and fax number.
